Semillas de Salud is excited to offer the John N. Hsia Memorial Scholarship for the academic year.

Applicant Eligibility Information

Please review the eligibility information for the scholarship. 

-High school senior or graduate.

- Planning to attend a four year college or university in pursuit of a bachelor's or post college graduate degree.

-Cumulative G.P.A. at least a 2.5 on a 4.0 scale during the last two years of high school or college.

- Intention of studying a program that will allow you to support or enhance health and wellbeing of the community.

-Demonstrate financial need

Grants

Grant Application Guidelines

Semillas de Salud Inc. is determined to make each gift envisioning a sense of purpose and clear expectation of measurable outcomes. 

 

Grant Requirements

All donor recipients must be qualified tax exempt organizations under section 501 ( C ) 3 of the U.S. Internal Revenue Service, or the equivalent federal designation in another country.
Grants will be given to organizations that demonstrate that they have competent management who have a history of delivering results in an effective and cost efficient manner. Levels of overhead expenses will be taken into consideration.
Grant recipients must have the ability and willingness to measure and report results.
We require self-reliance, responsibility and accountability.

Relevant materials about your organization and/or proposed project.
Letters of support from community-based groups or professional associations and evidence of networking will receive special consideration. 


Grant Limitations

Grants are generally rejected for contingencies, deficits or debt reduction.

Focus

The primary focus of Semillas de Salud will be to support any individual(s) and organization(s), in any discipline,  with a primary goal to contribute in the field of Health and Wellness.

Review Process
The Foundation reviews the following criteria in during the grant process. 

Clarity Of Purpose - The organization's request should be clearly defined with a specific description of how goals/tasks will be achieved and how funds will be spent. A succinct and pithy project description and budget must be included.

Potential For Success - The organization must demonstrate that projects are realistic and concrete, they have additional funding sources, manpower, and community support as well as the time to achieve their goals. 

Leadership - Stability in the leadership of the organizations is a requirement

Financial Stability - A history of overall financial stability is highly desirable. It is required that each organization provides their current audited financial statement, prior to funding.

Measurements And Results - All major gifts will require a semi-annual and a final report at the project's completion, outlining how the funds were spent, as well as detailing the overall achievements for the project.

Site Visits - While not always possible, site visits by board members prior to gifting, during the gifting period and following the project's completion are considered desirable.​

Date

Name of the organization

The name, mailing address, telephone, and email address of the responsible contact person including the link to the agency website.

Introduce the proposed program.

State the amount of funding being requested.  Applicants may request up to $25,000.

A statement that your organization’s governing board supports the grant proposal.

 

Program Details (limited to 800 words)

Provide specific details of the proposed program.  Do not duplicate information listed in the cover letter.

Note the geographic area and the target population to be served.

Describe the need and purpose of the program and the problems it addresses.

Provide the timeline needed to achieve the program’s goals.

Describe how the anticipated outcomes will be measured to evaluate the program.

Include information to describe your organization’s ability to carry out the program and mention other partners involved with the program.

List the personnel implementing the project.

 

Financials

Include a detailed agency budget.  Include the specific amount requested and how these funds will be allocated.

The current year operating budget with actual revenue and expenses to date.

Include the most recent fiscal year profit/loss statement.

Names of other public and private funders approached for support of this program and whether the amount requested has been received, is pending or was declined.

 

Organizational information (limited to 400 words)

Give a brief history of the organization.

Names of the organization’s officers and directors.

 

Addendum 

A copy of the organization’s IRS tax exemption letter.

A copy of the most recent form 990 and 199.
